A ‘medallion form’ refers to the form that requires a medallion signature guarantee stamp. This is the document you need stamped - typically a transfer form, stock power form, or other securities-related document.

Common Types of Medallion Forms

Stock/Securities Transfer Forms: Used to transfer ownership from one person to another

Stock Power Forms: Separate forms that authorize transfer of stock certificates

How to Prepare Your Medallion Form

Before sending us your form:

Complete all required fields - don’t leave anything blank

Sign in the designated signature area (NOT in the medallion guarantee box)

Include all pages of the form, even if some seem unimportant

Make sure the information matches your supporting documents exactly
