What you'll need.

A Medallion Signature Guarantee requires documentation. What you need depends on your specific situation — this page will help you prepare before you start.

The basics.

Every transaction requires three things. Have these ready before you start.

Government-issued photo ID

Valid, clearly readable, and reflecting your current address. Driver's license or passport.

Account statement

Dated within the past 6 months. Must show the shareholder's name, account number, type of stock, and number of shares.

Transfer forms

The document we'll be stamping. All pages, fully completed and signed. Usually provided by the transfer agent or brokerage requesting the Medallion.

Depending on your transaction type, you'll need additional documentation. Here's what to expect for the four situations we see most.

Individual

Transferring shares between living individuals — gifts, private sales, adding a joint owner, moving shares between your own accounts.

Estate

Most common

Transferring shares from a deceased person's estate to heirs or beneficiaries. The most common reason people come to us.

What you'll need

  • Government-issued photo ID
  • Account statement
  • Transfer forms
  • Death certificate
  • Letters testamentary or of administration

Or small estate affidavit, depending on estate size and state.

Common scenarios

  • — Inherited stocks from a deceased parent or spouse
  • — Executor transferring shares to beneficiaries
  • — Claiming securities as an heir

Trust

Transferring shares into, out of, or between trusts — or changing trustees.

What you'll need

  • Government-issued photo ID
  • Account statement
  • Transfer forms
  • Death certificate(s) (if applicable)
  • Complete, original trust document
  • All trust amendments

May also require trustee resignation or acceptance documents, or court orders in some cases.

Corporate & other

Transfers involving corporate entities, partnerships, or other ownership structures have unique requirements.

What you'll need

Varies significantly by situation. The questionnaire below will identify your specific requirements.
