Form Correction Policy

No Alterations Accepted

We cannot accept documents that have been altered in any way, including forms with any information that has been:

Crossed out or written over

Whited out (with correction fluid/tape)

Manually corrected/Modified with pen or pencil

Why We Can’t Accept Altered Forms

This strict policy exists because:

Transfer agents reject altered documents

Alterations raise fraud concerns

Changed documents may not be legally valid

Our guarantee cannot cover modified forms

Securities regulations prohibit accepting altered documents

What to Do If You Made a Mistake

If you’ve made an error on your form:

Obtain a fresh form from your broker or transfer agent

Complete the new form carefully

Double-check all information before signing

Submit the clean, unaltered form to us

Preventing Mistakes

Before filling out your form:

Read all instructions carefully

Gather all needed information first

Use black or blue ink

Write clearly and legibly

Have someone review it before signing

Updated Forms After Stamping

If you need to make changes after we’ve already stamped a form, you’ll need to start over with a new form and a new medallion guarantee. The same fee applies as this is a new transaction.
