Document Requirements

Standard Documentation for Individual Accounts

Basic Requirements for Personal Transfers

When you’re transferring securities or stocks in your own name, we need to verify three important things: your identity, your ownership of the assets, and your authorization to make the transfer.

Required Documents

Government-issued photo ID: Current driver’s license or passport

Account statement: Dated within the last 6 months, showing the securities

Transfer forms: Completely filled out and signed (but not in the medallion box)

*Please note that each individual transfer request is subject to additional documentation and/or a Zoom meeting. *

About Your Account Statement

Your account statement is required as proof of ownership. **It must be dated within the last six months **so we can confirm the information is up to date. Think of it like providing a recent utility bill to verify your address but here, it verifies that you own the securities you’re transferring.

The statement should clearly show:

Your name (matching your ID). In certain scenarios, proof that you are the registered owner

The securities you’re transferring

The account number

A date within the last 6 months

Signing Your Medallion Form

The medallion form itself needs special attention. These forms must be completely filled out with all required information, and most importantly, you need to sign them in the specific spot designated for your signature - NOT in the medallion guarantee box.

The medallion guarantee area is usually marked clearly on the form, often with text like ‘Signature Guaranteed’ or a box labeled for the medallion stamp. Sign right next to this box, not inside it. We’ll add our stamp and signature in that special area.

Please note that these are the basic documents we require to initiate the transaction, and we reserve the right to request additional documentation to ensure your transaction can be processed.
